Social Security numbers and birth dates for everyone claimed on your tax return

Income Items

  • W-2's and all 1099's received
  • 1099INT for interest income
  • 1099DIV for dividend income
  • 1099B for sales of securities (provide cost for each item sold)
  • 1099R for annuities, pension IRA's and KEOGH income
  • 1099G for state tax refunds
  • SSA 1099 for Social Security income
  • 1099G for unemployment income
  • 1099 MISC for non-employee compensation, rent income or other miscellaneous income
  • K-1 for partnerships, joint, ventures, S corporations, estates, and trusts
  • Alimony received
  • Jury duty pay
  • Gambling and lottery winnings
  • Prizes and awards
  • Scholarships and fellowships
  • Tips not reported on W-2

Deductions

  • Student loan interest paid
  • Early withdrawal penalties on CD's and other time deposits
  • Contributions to IRA's, KEOGH, simple retirement plans

Medical Expenses

  • Health Insurance, Long Term Care Insurance Premiums, Vision Insurance, Dental Insurance
  • Prescription Drugs
  • Doctors and Clinics, Dental and Orthodontist, Hospital
  • Glasses, Contact Lenses, Eye Exams, Laser Eye Surgery
  • Lab Tests, Therapy, X-Ray, Anesthesiology
  • Prescribed Medical Equipment: Hearing Aids, Batteries, and related costs
  • Stop Smoking programs, Weight Loss Programs
  • Medical Miles at $0.14 cents a mile (other medical transportation, ambulance, taxi, bus and lodging while obtaining medical care) limited to $50 per night

Taxes

  • State or city tax withheld from wages

Interest

  • First and second mortgage interest, home equity, home improvement, loan points
  • Contract for deed (seller financed) - must list name, address and SSN

Contributions

  • Churches, Synagogues
  • United Way Campaign, Cancer, Heart, Scouts, MS, MD, March of Dimes, Food Shelf
  • Out of pocket expenses for charitable work
  • Value of goods donated to Goodwill, Salvation Army, etc.
  • Charitable mileage on auto at $0.14 cents per mile

Miscellaneous

  • Non-reimbursed employee expenses (union dues, tools, supplies, special uniforms, safety equipment, professional dues and subscriptions, job related education)
  • Gambling and lottery losses

Investment related

  • Phone, postage, supplies for investments
  • Safe deposit box
  • Investment publications
  • IRA and KEOGH fees paid directly by you
